I don’t know what you use, but I swear by Spam Karma 2, with the addition of Bad Behavior (sorry about the American spelling, not my fault) which blocks a lot of things before they even get here.
Since installing BB I’ve had no spam at all. If you haven’t got it, also put in CJD’s Spam Nuke which lists all the items in the WP database tagged as spam, so you can make sure nothing gets deleted incorrectly.
